This Package is designed for families of older adults or older adults themselves who want end-to-end support navigating the aged care system - from assessment through to service setup and handover to a provider. You want someone experienced, compassionate, and organised to walk alongside you the whole way and take the lead where needed.
Includes
Intake & Goal Setting
- A Face-to Face or virtual meeting with the older person and/or their family
- Clarify current challenges, wishes and care goals
- Determine eligibility and readiness for My Aged Care Referral
- Referral submitted on your behalf if needed
Assessment Preparation
- Preparation sheet summarising needs and key talking points
- Practical coaching for family members on how to support the conversation
- Assistance at the My Aged Care Assessment (face to face to virtual)
- Guidance on advocacy for additional supports if needed
Support Plan & Notice of Decision Review
- Detailed walk through of your support plan and notice of decision
- Clear explanation of funding codes, eligibility, and timelines
- Identify CHSP codes and services that can be utilised as interim supports
- Setting up CHSP services as required with local support services until funding is available
Tailored Provider Matching ​
- Provider recommendations aligned with your needs, values and goals of support
- Request information packs and availability of sign up
- Assist with decision making and comparisons
- Be present at intake visit with new provider (in person or virtually)
-In depth handover of your journey to your chosen provider
Ongoing Support & Check Ins
- Available to assist you with any questions that may arise from intake to handover
- Priority response to emails and phone call support - answering you as quick as we can to put your mind at ease and support you through your journey.
