It's perfect if you need guidance around eligibility, services that are available and basic information on navigating the My aged Care System.
Includes
Initial Discovery Session
- A Face-to Face or virtual meeting with the older person and/or their family
- Explore goals, needs, preferences and concerns
- Identify early signs of unmet needs and care gaps
Assessment Preparation Support
- Help preparing and submitting a referral to My Aged Care
- Guidance on understanding the language used at assessment so you are empowered to have your concerns voiced.
Assessment Planning
- Coaching on what to expect during the assessment
- Support to highlight key issues that need to be discussed with the assessor at the assessment

- A summary of needs and goals to take into the assessment
- Clear written guidance on next steps
Phone & Email Support
- 2 Weeks of support from initial discovery to answer any questions that may arise
