We will guide you through each step while you take the lead.

Initial Consultation
- 1:1 (in person or virtual) meeting to understand your goals, concerns & current supports
- Family involvement encouraged and supported
- A chance to clarify priorities and next steps
Assessment Support
- Help preparing and submitting a referral to My Aged Care
- Guidance on understanding the language used at assessment so you are empowered to have your concerns voiced.
​Set-Up Interim Support Services
- We will sit with you to assist you in understanding your support plan and your notice of decision, breaking down the jargon so you can plan your next steps.
- Help you understand the CHSP codes that may have been initiated to access short term supports
- Guidance on where to find these CHSP providers and how to activate the codes to utilise services
Tailored Provider Matching
- Shortlist of quality providers that fit your needs, values and goals when it comes to support
- Assistance requesting information packs and comparing offers
Short Term Check-Ins
-Support from initial consult to assessment (monthly check ins until assessment date known)
- Phone call prior to assessment to clarify any last minute questions
- Phone or email support throughout the entire journey until commencement of support services with provider.
